Action Required: Complete Bridge Access Program (BAP) Enrollment
Dear Provider,
According to our records, you have been selected by CDPH to participate in the COVID-19 Bridge Access Program (BAP) but have not completed your enrollment in myCAvax. You must complete the enrollment process to order COVID-19 vaccines through BAP on myCAvax.
To enroll, your organization's coordinator and/or primary vaccine coordinator should:
- Log in to myCAvax.
- Complete the required BAP program training from the ‘Training’ tab in myCAvax.
- Select your COVID-19 program location from the pop-up that appears on the homepage.
- Provide the required information and upload a copy of the signed BAP Provider Agreement.
- Click ‘Submit’, and your COVID-19 location will be placed in ‘Active’ status.
NOTE: If you have multiple locations eligible to participate in the Bridge Access Program, you will need to enroll each location individually.
To update the contact information of new organization coordinator(s) and/or new primary vaccine coordinator(s), email the Provider Call Center at ProviderCallCenter@cdph.ca.gov or call (833) 502-1245, Monday-Friday, 8AM-5PM.
